Triple Aim Health Foundation provides charitable financial assistance to eligible Medicare Part D patients who need help with out-of-pocket costs for certain provider-administered prescription medications.
Providers and care teams can help identify potential candidates, assist with applications, and provide clinical information when needed.
A patient may be eligible if they:
Triple Aim Health Foundation independently determines eligibility. Meeting the initial requirements does not guarantee assistance.
If a Medicare Part D patient is having difficulty affording a qualifying provider-administered prescription, your team may refer the patient to Triple Aim or assist with the application.
A healthcare provider, clinic representative, pharmacy representative, family member, caregiver, or other appropriately authorized representative may help complete an application on the patient’s behalf.
Applications include information about the patient’s Medicare Part D coverage, diagnosis, prescribed medication, household income, and prescribing provider.
Triple Aim may contact the prescribing office to verify information relevant to eligibility, including the patient’s diagnosis, prescribed medication, prescribing provider, or other clinical details.
Clinic and provider staff may provide an ICD-10-CM code when available.
Triple Aim provides financial assistance and does not prescribe medications, recommend treatments, or make medical decisions.
Treatment decisions remain between the patient and their healthcare provider. Eligibility and award decisions are made independently by Triple Aim and are not conditioned on the use of a particular provider, clinic, pharmacy, manufacturer, product, referral source, donor, or funding source.
