Patient Assistance Information

When a patient applies for or receives financial assistance, we may collect information necessary to determine eligibility and administer the assistance program, including:

  • Medicare Part D and other relevant insurance information
  • Diagnosis or medical condition relevant to program eligibility

Information From Healthcare Providers, Pharmacies, and Others

We may receive information about an applicant or assistance recipient from healthcare providers, pharmacies, authorized representatives, program administrators, or other parties involved in the application or assistance process.

For example, we may receive information necessary to confirm a patient’s diagnosis, prescribed treatment, insurance coverage, qualifying out-of-pocket costs, prescription fills, claims, or use of an assistance award.

Donor Information

When you make a donation or otherwise support Triple Aim, we may collect information such as your:

Triple Aim independently determines eligibility for patient assistance and the amount of assistance provided under its established program requirements and available funding.

We do not provide donors with identifiable patient information for the purpose of allowing a donor to select individual patients, determine an individual’s eligibility or award amount, influence the choice of healthcare provider, pharmacy, or prescribed treatment, or market products or services to patients.

Triple Aim may provide donors and other funding partners with aggregated, de-identified, financial, or program-level information for legitimate reporting, compliance, stewardship, and organizational purposes.

Donor Privacy

Triple Aim does not sell, rent, or trade donor personal information.

We do not provide donor information to other organizations for their independent marketing purposes.

Donor information may be provided to payment processors, fundraising platforms, accountants, professional advisors, and other service providers when necessary to process donations or administer Triple Aim’s charitable operations.

Sensitive Information

Because patient assistance requires us to evaluate financial, insurance, diagnosis, prescription, and treatment information, Triple Aim may need to collect sensitive personal information.

We seek to collect only information reasonably necessary to determine eligibility and administer assistance.

We do not use patient diagnosis, prescription, insurance, income, or other sensitive application information for targeted advertising.

Where consent for the collection or processing of sensitive personal information is required by applicable law, Triple Aim will obtain that consent through the applicable application, authorization, or other appropriate process.
