Fund Definition

The Autoimmune & Inflammatory Disease Fund provides financial assistance toward eligible out-of-pocket prescription costs for Medicare Part D patients receiving qualifying provider-administered treatment for autoimmune, inflammatory, and related conditions.

Do I Qualify?

To qualify for assistance through the Autoimmune & Inflammatory Disease Fund, patients must:

  • Be enrolled in Medicare Part D
  • Have a qualifying diagnosis
  • Be prescribed a qualifying provider-administered medication
  • Have household income at or below 300% of the Federal Poverty Level
  • Live in the United States or a U.S. territory
  • Receive the requested treatment in the United States or a U.S. territory
  • Meet all other current Fund requirements

Submitting an application does not guarantee assistance. Triple Aim Health Foundation independently determines eligibility based on current program requirements, the information and documentation submitted, and available funding.
