To qualify, a patient generally must meet all of the following requirements:
Are you enrolled in Medicare Part D?
You must be enrolled in Medicare Part D and meet the program’s insurance requirements.
Do you have an eligible diagnosis and prescribed treatment?
You must have a qualifying diagnosis and a valid prescription for an eligible specialty treatment.
Does your household income fall within our guidelines?
Household income must generally be at or below 300% of the Federal Poverty Level.

You must reside and receive treatment in the United States or a U.S. territory. U.S. citizenship is not required.
Are you responsible for qualifying out-of-pocket costs?
You must be responsible for qualifying out-of-pocket costs associated with the prescribed treatment.
Meeting eligibility requirements does not guarantee approval. Final eligibility is determined after review of a complete application and required supporting documentation. Assistance is subject to available funding.
Is your prescribed treatment potentially eligible?
Search for your prescribed treatment to see whether it has previously been supported. A treatment’s appearance in the results does not guarantee eligibility.
Try a generic name, such as infliximab, or a brand name, such as Remicade.

Treatments previously supported by Triple Aim have been used across several areas of care, including:
- Rheumatologic and autoimmune conditions
- Inflammatory bowel diseases
- Immunologic conditions
- Allergic and inflammatory conditions
- Osteoporosis and certain bone-health conditions

When applications are open, a patient or authorized person may submit the required information and supporting documentation.
Triple Aim reviews eligibility
Triple Aim reviews completed applications under the program’s eligibility requirements.
Receive assistance
If approved, a patient may receive financial assistance toward qualifying out-of-pocket costs, subject to award terms and available funding.
Approval does not guarantee payment of every treatment expense. Assistance is limited to eligible expenses and the available award amount.
Your treatment decisions remain yours
Triple Aim Health Foundation does not prescribe treatment or make medical decisions. Those decisions remain between patients and their healthcare providers. Triple Aim independently determines eligibility and assistance awards under established program requirements and available funding. Donors do not select individual recipients or determine eligibility or award amounts.

Eligible patients may receive up to $2,100 in annual assistance, subject to program requirements, award terms, and available funding.
Yes. Your healthcare provider or care team may help you understand the requirements, prepare documentation, and provide relevant diagnosis, prescription, and treatment information. Triple Aim makes the final eligibility determination.
When applications are open, follow the instructions provided with the application for submitting information on a patient’s behalf.
Treatment decisions remain between you and your healthcare provider. Triple Aim does not prescribe treatment or make medical decisions.
