Based in Aspen, Colorado, Triple Aim Health Foundation was created to help close the gap between prescribed care and affordability. We provide financial assistance toward qualifying treatment costs for eligible patients.

A patient generally must meet all of the following requirements:
The patient must be enrolled in Medicare Part D and meet the insurance requirements of the program.
The patient must have a qualifying diagnosis and a valid prescription for an eligible specialty treatment.
Household income must generally be at or below 300% of the Federal Poverty Level. Federal Poverty Guidelines are updated annually.
The patient must be responsible for qualifying out-of-pocket costs associated with prescribed treatment.
