Triple Aim Health Foundation helps reduce the financial barriers that can interrupt medically necessary care, so patients can start, sustain, and continue the treatment they depend on.

How to apply

Our patient assistance grants provide fast and reliable help to pay for medication copays.
Enroll your pharmacy
Dispensing healthcare providers & pharmacies must enroll their pharmacy ahead of submitting a patient application.
Submit a patient application
Be prepared with the right information and documents for your application.
Patients are eligible for Triple Aim Foundation Assistance if:
Reside in the US
The patient resides and is receiving treatment in the United States or U.S. territories. (U.S. citizenship is not a requirement)
Are covered by Medicare Part D
The patient must be covered by Medicare Part D. This program is aimed at lowering patient financial toxicity.
Have a valid prescription
The patient must have a valid prescription for a course of immunology provider-administered medications for treatment of a documented medical condition.
Meet income requirements
The patient’s annual income must be below 300% of the Federal Poverty Line (FPL). The U.S. Federal Poverty Guidelines change every year; current guidelines are available here.
